- Information on obtaining research approvals from IACUC and UCRIHS
Approval for use of human subjects in University research is required by the University Committee on Research Involving Human Subjects (UCRIHS). Approval for the use of vertebrate animals in University research is required by the All-University Committee on Animal Use and Care (AUCAUC). Copies of all relevant approval forms must be submitted with your thesis if you check the “vertebrate animals” or “human subjects” boxes on the Report of the Guidance Committee forms. For more information, please contact your advisor or refer to the AUCAUC and UCRIHS websites.

- Forms required for completion of the Ph.D. degree
The Graduate School website has copies of the forms required for the completion of the Ph.D. degree for doctoral students. Links to several of these forms are included below. The first form is filled out when the student completes both oral and written sections of the Comprehensive Examination. The second form is submitted two weeks before the dissertation defense. The third form is submitted after the dissertation defense. All forms are submitted to the Graduate Secretary in the Zoology office. The documents are published as Microsoft Word .DOC files.
